I tested importing one new photo into LR since the upgrade and I have not changed the destination. The file paths to photos are not relative to the catalog location, so moving the catalog doesn't create any problems in that respect. If you re-import photos where you had written metadata to file, and removed them from Lightroom, you can restore some metadata after import, by selecting the files, and going to Metadata > Read metadata from file.

I suspect Lightroom Catalog was your first one, Lightroom Catalog-2 was the next upgrade, Lightroom Catalog-3 was the upgrade after that, and Lightroom 5 Catalog was a separate catalog created at some stage.
